Case Management

Case management is the process by which a professional, either a nurse or social worker, assists the patient in meeting his or her specific needs. The case manager works with the patient to identify those needs, and the necessary steps to meet them. Case management also allows the patient to have someone on his or her side when it comes to making decisions that affect the individual's health and well-being. At Vandenberg Space Force Base, the case manager may also provide short term care for those having difficulty finding the right specialist for their particular needs. Case Management services are available to patients who are eligible for treatment at the 30th Medical Group.
